Special Dietary Needs Procedure

Students with

Special Dietary Needs

 

~ Special dietary needs form (see form on next tab) must be completed & signed by a physician

~ Form is to be returned to school & sent to Child Nutrition Director

~ Child Nutrition Director will work with the cafeteria manager, school nurse & doctor to meet the need of accommodations for the student

~ A copy of the special diet form will be kept in the CND office & cafeteria
